JOB DESCRIPTIONFAMILY TIES OF WESTCHESTERCASE PLANNEREVERY KID COUNTS PROGRAMHOURS OF WORK: Full time - 35 hours per week.POSITION SUMMARY: Under general supervision, the Every Kid Counts (EKC) Case Planner provides services to children and their families to improve their functioning and well-being. The EKC Case Planner provides services to low and moderate risk families who are referred to the program by the Westchester County Department of Social Services Child Protective Services and Cross Systems Units. The position is located in White Plains and serves families who live throughout Westchester County. The Every Kid Counts Case Planner assesses the need for services and arranges for the provision of such services, either personally where practicable, or by referrals to other appropriate service providers.The goal of the Every Kid Counts Case Planner is to engage the family in a process that enables them to develop and implement a service plan that strengthens the family, prevents their children from being placed into foster care and prevents future cps reports once the case is closed. Families are introduced to their local Family Ties Resource Center, where they may continue to return for assistance, long after their Every Kid Counts case is closed.The EKC Case Planner is a full and active team member of Family Ties.Willingness to work flexible hours is required to accommodate working families and to participate in support groups with parents, parent trainings, and Family Ties functions, as needed.The services provided are performed utilizing the CASSP Principles and Westchester System of Care principles and values.Full time; Salary $55,500. Some nights may be required EXAMPLES OF WORK: (Illustrative Only)1.Establishes and maintains at least weekly contact with families; most contacts are in-home contacts at times convenient for the family;2.Carefully monitors risk and safety throughout the families’ involvement with Every Kid Counts; The l EKC Case Planner is a mandated reporter;3.Introduces families to the wraparound process through the Family Guide to Wraparound;4.Conducts an evaluation of families' strengths and need for services utilizing the Child and Adolescent Needs and Strengths (CANS) Comprehensive Assessment and the FRIENDS Protective Factors Survey, and develops an appropriate service plan with the family within the required time frame; 5.Facilitates wraparound planning meetings using the Family Unity/Network model where appropriate and helps develop an ongoing Support Circle for the family if needed;6.Provides parent coaching/training utilizing Triple P Parenting for which EKC staff are trained if not already certified in this model;7.With the families' permission, makes necessary collateral contacts; 8.Carries a caseload of six (6) to eight (8) Families at any given time;9.Completes engagement, assessment, service provision, referrals, and case closing within 3-6 months as appropriate for each family;10.Keeps necessary records, maintains timely documentation in the electronic record, provides reports, and statistical information to Family Ties and DSS as required;11.Acts as an advocate on behalf of families and may attend a variety of meetings such as CSE meetings with the family when requested to do so12.Appears in Family Court when requested to do so, either as an advocate for the family, or when required to present testimony;13.Attends meetings, conferences, and trainings as required;14.Participates in evaluation activities;15.Uses computer applications or other automated systems in performing work assignments. QUALIFICATIONS: Bilingual (English/Spanish) preferred. Minimum of a BSW or alternately a Bachelor's degree and two years’ experience working directly with families and children providing social work and case planning/management services, or an Associate’s degree with four years’ experience working directly with families and children providing social work and case planning/management services. Master's Degree in a human service field preferred. Candidates must possess good writing skills along with, strong computer, verbal and organizational skills in English (and Spanish). Willingness to work flexible hours is required to accommodate working families and to participate in evening support groups and parent trainings as needed. Flexibility and ability to be a team player is essential along with the ability to develop positive relationships with Department of Social Services Staff and community agencies. Candidates who are themselves parents are preferred. Valid driver's license and car required. All candidates must receive satisfactory clearances from the Child Abuse Registry and Criminal History Information Tracking System.
