FULL-TIME Nonexempt : 35 hours per week

Coordinated Care Services Inc (CCSI) (https://positions.paylocity.com/PositionManagement/wizard/2033530) seeks a talentedAssistant Director for Adult Servicesto contribute their expertise in providing essential business services to our provider organizations and customers. The Assistant Director for Adult Services is assigned to work on behalf of CCSI’s customer and will work closely with the Wyoming County Director of Community Services to develop, plan, oversee, and coordinate local adult mental health, substance use disorder, and intellectual/developmental disability services provided through contract and other agencies, especially those for high/risk, high/need and seriously mental ill adults 18 years and over. Advocates for and promotes services that enhance recovery and rehabilitation; leads Single Point of Access (SPOA) services for adults; manages all Assisted Outpatient Treatment (AOT) services in Wyoming County; coordinates the Suicide Prevention Coalition and community education efforts.

In this role, you will

1) Serve as SPOA coordinator. Assist with and oversees care coordination and development of care plans for adult high-risk high-priority individuals with mental illness, especially those individuals who are hospitalized, homeless, dually diagnosed (mental illness along with substance use disorder or developmentally disabled), in the criminal justice system, or otherwise at risk.

a. Chair or co-chair weekly SPOA meetings.

b. Monitor access to various case management, including care coordination, housing, peer, self-help, and other needed services.

c. Maintain the county priority that persons of acute risk are given rapid access to services, with those whose situations have begun to stabilize moving on to lower levels of care if necessary.

d. Advocate for appropriate level of care and service utilization and reports gaps in service to the Director of Community Services (DCS).

e. Maintain tracking system to monitor client status and progress.

f. Monitor roster, referrals, client status, provider agreements, develop referral documentation and process, and develop agendas for meetings

g. Assure that New York State Office of Mental Health reporting requirements are met.

h. Assist with establishing training programs which will benefit individuals with mental illness, substance use disorders and intellectual/developmental disabilities

i. Represent the Mental Health Dept. in various county meetings.

j. Serve as DCS designee for 9.45 police transports when necessary

2) Lead and maintain all AOT-related (Assisted Outpatient Treatment) services in Wyoming County, including referral and eligibility, assuring that client rights are protected, setting up court involvement and treatment plans, documentation, monitoring progress, coordinating 9.60 orders if needed, and full reporting to the Office of Mental Health and other parties.

3) Chair of the Wyoming County Suicide Prevention Coalition

4) Coordinate/provide community education related to mental wellness, treatment/recovery in Wyoming County.

5) Serve as liaison between the Mental Health Department and Wyoming County peer support recovery programs.

6) Participate in, track, and coordinate Wyoming County forensic activities including, Crisis Intervention Team and coordinate outreach to high-risk individuals in the criminal justice system and report to sheriff and other local departments as needed.

7) Serve in an advisory role for the Wyoming County First Responder Support Team once trained in critical incident/disaster response trainings

8) Coordinate or assist with special projects as assigned by the DCS (i.e. May is Mental Health Awareness Month activities).

9) Perform other duties as assigned.

Requirements

  • Licensed Practitioner of the Healing Arts (such as Registered Professional Nurse, Licensed Social Worker, Licensed Clinical Psychologist; Physician’s assistant; Psychiatric Nurse Practitioner) with a minimum of 5 years progressive experience in community-based mental health services or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) ).

Additional Requirements

  • Satisfactory completion of all required background screenings.

  • Familiarity with New York State Office of Mental Health and Wyoming County public mental health system preferred.

  • Must have initiative, ability to work independently and as part of a leadership team, strong leadership abilities, and excellent interpersonal skills.
